At Reyna Psychiatry PLLC, I work primarily with patients between the ages 4-20 years old. However, appointments for older adults may be available.

Please note that only the patient or legal guardian may complete this form, and it does not guarantee an appointment nor establish a patient-doctor relationship. We also do not accept any type of insurance and cannot see patients with Medicaid or Medicare (even if they are secondary insurance).
